Veteran Nursing provides a simple referral pathway for GP clinics to support their veteran and war widow patients through simple in-home nursing assessment. The process is straightforward, requiring only a D904 referral (via email) and a current health summary to initiate the process.
Once referred, an in-home assessment is completed with the veteran or war widow within their home, with practical actions taken where appropriate and within scope. This may include basic support to apply for MyAgedCare assessment, home use health equipment, incontinence product arrangement and consideration towards falls support alarms/equipment.
A clear, concise report is provided back to the GP outlining key findings and practical recommendations for veteran centric ongoing care. This will often include suggestions to engage additional local services that may not have been previously considered, as well as guidance on accessing appropriate advocacy support for non accepted or sequela conditions. Where relevant, this may also highlight opportunities to better align existing management with available DVA supports and pathways. This process supports improved patient outcomes, reduces the risk of hospitalisation, and strengthens the GP’s ability to effectively deliver the Coordinated Veterans’ Care program.
Additional support can also be provided to clinics on an as needed basis to improve identification of veterans within the patient population and build confidence in navigating DVA processes, referrals, and available supports.
